Virtual Follow-Up Appointments Available

FUNCTIONAL MEDICINE NEW PATIENTS are required to be seen in office for your first appointment and virtual appointments are offered, but not required, for follow-up appointments.  All virtual appointments are through our HIPAA compliant, easy-to-use virtual platform. Your virtual appointment is easily accessed by using a smartphone (iPhone or Android), tablet or computer.

ALLERGY CLINIC NEW PATIENTS can be seen in office or virtually for your first appointment.  Virtual appointments for ALLERGY CLINIC FOLLOW-UP appointments are offered, but not required.

Once you've established your appointment with us, you'll be asked to register on our patient portal. At the time of your virtual appointment, you will login to your patient portal account and visit the "Appointment" section located on your dashboard.  You will then select "Check-in" next to a video camera icon. This will prompt instructions to finalize your secure connection via ZOOM video platform.

If you have any prior lab work or medical records, you can upload them in your secure patient portal as PDF files in "Documents" and select the provider you'd like your records to be shared with. You may also have your records faxed to us in advance at 888-394-6442.

We have provided a medical release of information form below to assist you in obtaining your medical records.

New Functional Medicine Adult Patients

Once you schedule an appointment with us, we will send you new patient forms through your secure online patient portal. Please take your time to read and complete these materials. New patients are asked to fill out the New Patient Registration and Consent Forms, the New Patient Lifestyle Questionnaire, the Self Care Quesionnaire and the Multiple System Questionnaire and submit these prior to your initial visit.

Please allow 2 hours for your initial visit, which will include approximately 1.5 hours of in-depth consultation, review of medical records and physical exam by your functional medicine physician and approximately 20 to 30 mintues of consultation with your health coach. We will review with you the procedures for obtaining initial diagnostic laboratory testing if needed at your initial visit.

New Functional Medicine Pediatric Patients (ages 13 -17)

Once you schedule an appointment with us, we will send new pediatric patient forms through your secure patient portal.   A legal guardian must complete the New Pediatric Consent and Registrations forms and submit these prior to the new pediatric patient visit.  

New Adult & Pediatric Allergy Drop Patients

Once you schedule an appointment with us, we will send you new patient forms through your secure online patient portal. Please take your time to read and complete these materials. New patients are asked to fill out the Allergy Patient Registration Form.  

Please allow 30 minutes for your initial visit.  We will review with you the results of any blood allergy testing OR review what tests need to be completed.  Once test results are available we will provide you with a prescription for allergy drops.  If needed, we will also discuss how and when to obtain blood testing for food allergies. 

Insurance Coverage

Our office does not bill your insurance directly, but we will provide you with a coded receipt for services, which you may submit for reimbursement to your insurance carrier. All of our physicians are out-of-network providers for all private insurance plans and do not participate with Medicare or Medicaid. The amount of reimbursement you receive depends on your insurance plan's reimbursement policy for out-of -network providers, your deductibles, copays and co-insurance. You may wish to check with your insurance plan regarding out-of-network coverage of services prior to your visit with us.

We will request your insurance information for ordering labs in your initial Registration Questionnaire.

Services associated with our clinic including provider visits, allergy testing, counseling, supplements and laboratory testing can be submitted under your health savings account (HSA) or Healthcare Reimbursement Account (HRA).

Fees & Payment

Carolina Total Wellness operates as a self-pay clinic. We accept cash, checks or credit cards for services rendered. Payment is expected at time of service.

Cancellation Policy

If it is necessary for you to cancel or reschedule your appointment, we require that you call the office with at least 2 business days notice. To cancel an appointment, please call 919-999-0831.

Late Cancellations or No Show

We understand that unexpected changes of schedule occur and allow for an occssional missed appointment. After 2 late cancellations or 3 consecutive cancellations (including rescheduling and no-shows) a 50% appointment fee is required prior to scheduling another appointment.
